Special Needs Requests:

Indicate any special needs you may have during the conference because of physical disabilities or sensory impairment (please submit requests by July 22). Use additional sheets, if necessary.

Cancellation of Registration

Cancellations or substitutions for registrations must be in writing and emailed or faxed to the Conference Registrar before July 22nd. Because conference registration is limited and work is performed once registration is processed, there is a $50 administrative fee for any cancelled registration.
