Early registration is strongly recommended, as space may not be available at the time of onsite registration.
Registration includes: Four days of training consisting of Monday’s Opening Lunch with CADCA, general plenary sessions, breakout sessions and Thursday’s Closing Keynote. Friday sessions and In-Depth Trainings are separate fees.

DAILY RATES
You may register to attend the Institute by Day (Monday, Tuesday, Wednesday, Thursday or Friday)
DAILY Rates on or before June 15 – $125
DAILY Rates Starting June 16 – $150
Registration is not considered complete until payment is made in full. PAYMENT MUST BE RECEIVED BY Monday, July 22. Individuals with outstanding balances cannot attend sessions.
Individuals who submit a registration form indicate they have read and understand all payment, cancellation and refund policies pertaining to this registration.
Cancellation of Registration
Cancellations must be in writing to the Institute Conference Registrar and postmarked by Friday, June 28, 2013. No verbal cancellations are accepted. No cancellations may be reinstated. All cancellations requested to be reinstated will be handled as a new registration, subject to the conditions at the time the registration is requested. A $50 administrative fee will be deducted from all refunds. No refunds will be allowed after June 28. Send notification to:

Substitutions
