Registration is not considered complete until payment is made in full. PAYMENT MUST BE RECEIVED BY Monday, July 14. Individuals with outstanding balances cannot attend sessions.

Special Needs Requests:

Indicate any special needs you may have during the conference because of physical disabilities or sensory impairment (deadline, July 1st). Use additional sheets, if necessary.

Cancellation of Registration

Cancellations or substitutions for registrations must be in writing and emailed or faxed to the Conference Registrar before June 27. Because conference registration is limited and work is performed once registration is processed, there is a $50 administrative fee for any cancelled registration.

All cancellations are subject to a $50 administrative fee. After June 27th no refunds will be issued and payment is due in full. Send notification to:
